Ambulatory Care Pharmacist
Franciscan Healthcare is looking for an Ambulatory Care Pharmacist to provide pharmaceutical care and clinical services to patients in an ambulatory care setting. The primary focus will include medication management, patient education, and collaborative drug therapy practice. You will work in partnership with physicians, nurses, and other healthcare professionals to ensure safe and cost-effective plans that lead to optimal patient outcomes.

Hours: Full-time, Exempt
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Perform comprehensive medication reviews and assessments to ensure appropriate and safe medication therapy for patients.
- Provide clear and concise medication counseling to patients.
- Work closely with healthcare teams to ensure coordinated and comprehensive care for patients.
- Participate in quality improvement initiatives to enhance patient safety, medication-related outcomes, and overall healthcare delivery.
- Maintain accurate and up-to-date patient records. Ensure compliance with legal and regulatory requirements.
QUALIFICATIONS:
Education and/or Experience
- Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy
- Active or ability to obtain an unrestricted pharmacy license in the state of Nebraska
- Completion of a postgraduate residency program in ambulatory care pharmacy or equivalent experience, preferred
- Strong clinical knowledge and understanding of ambulatory care pharmacy practice
