Our Health Information Management Department is here to help you when it comes to accessing your medical records. The department is open Monday through Friday from 8 a.m. to 4:30 p.m.
Request for Access to Protected Health Information
Please complete the Request for Access to Protected Health Information form to have a copy of your medical records sent to you.
Authorization for Release of Information
In order for Franciscan Healthcare to release medical records to anyone other than an individual patient, the patient must complete the Authorization for Release of Information form.
